Latham Centers Inc. Assistant Supported Community Living Mgr in Yarmouth Port, Massachusetts

Plan & provide direct support services to individuals living in shared living or independently in the community. Your primary responsibilities will include coordinating support with shared living providers or individuals, managing monthly financial needs, medical appointments, shopping, & other personal needs. Additionally, you will be responsible for coordinating & completing required documentation for the Department of Developmental Services (DDS) in support of the individual\'s Individual Support Plan (ISP) & medication treatment plan. This position requires flexible scheduling & on-call coverage as part of the Supported Community Living Management Rotation. Key Responsibilities: Work with a SCL Manager to support all functions that attain & maintain accreditation standards &/or program compliance with state regulatory agencies. Provide non-violent crisis intervention & support as needed. Assist a SCL Manager with program development, implementation, & design. Review monthly financial packets for submission according to established procedures, timelines & oversee individuals\' financial records & money management plan Conduct & document regular & ongoing home visits with individuals & shared living providers to ensure a safe, healthy, & positive living environment Coordinate with designated medical personnel to ensure the provision of medication medical care oversight functions consistent with DDS/MAPs requirements. Provide annual fire safety & human rights training to shared living individuals. Complete comprehensive, annual assessment of all shared living placements Document case management activities & all reportable behavioral incidents &/ or reportable events. Assist & support individuals with their ADL skills, personal needs & community involvement. Requirements: Bachelor\'s degree in a related field (Social Work, Psychology, Human Services, etc.) or equivalent experience- Previous experience in providing direct support services to individuals with developmental disabilities or related populations is preferred Strong organizational & time management skills Excellent communication & interpersonal skills Ability to work independently & as part of a team Flexibility in scheduling, including on-call coverage Latham Centers, Inc.
