Advocates Nurse Manager in Woburn, Massachusetts

Overview

Starting rate $47.00/hour

The Nurse Manager will oversee and coordinate all health care related activities across the DS/BI Northeast (NE) residential programs. The incumbent ensures adherence to varying contractual requirements by program as it pertains to clinical and allied health care functions. They will supervise and support the LPN’s working in these programs, be a MAP consultant, trainer, and provide nursing coverage where necessary.

Responsibilities

  1. Provide supervision, mentoring, and professional development opportunities for LPN nursing staff in the NE residential programs.

  2. Ensure all nursing, MAP, health care related policies and procedures meet compliance standards. for the NE residential programs.

  3. Serve as a member of the residential supports referral team to advise on all healthcare related and nursing needs for all potential referrals.

  4. Coordinate all high medical risk in conjunction with Director of Nursing/DS/BI residential leadership team.

  5. Coordinate and attend monthly nurse team meetings.

  6. Provide on call assistance to LPN’s and program staff regarding any medication and health related issues.

  7. Oversee the LPN follow-up instructions to program staff involved in medication occurrences.

  8. Maintain high quality of health service operations standards while maximizing cost efficacy.

  9. Teach all RN required individual medical protocol trainings for the NE.

  10. Follow up on DPH registered programs and their med certified staff to ensure that MAP guidelines for the administration, documentation, storage, and disposal of medications are being followed.

  11. Provide trainings to the LPNs at the NE residential programs that focus on taking clients to medical appointments and how to communicate effectively with healthcare providers.

  12. Present any medical related trainings at the monthly management meetings.

  13. Network with other health care professionals and providers within program communities.

  14. Assist to streamline communications between agency and the DDS regarding health and safety issues by:

  15. Developing a standard of in-house record keeping and reporting of incidents, behavioral episodes, and any other day to day monitoring of acute and chronic health related issues.

  16. Provide as needed instructions on how to put together the “personal well being” piece of the ISP for individuals with complex health issues.

  17. Teach LPN’s who attend ISP meetings on how to present medical data and to communicate effectively with DDS service coordinators.

  18. Provide on-site support to LPN’s, direct care staff and program managers by assisting in the development of strategies to address the special needs of clients with a variety of acute and chronic health issues.

  19. Provide direct nursing care as needed and when other generic health care systems have been exhausted.

  20. Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights policy and the Advocates Way

  21. Attends and actively participates in supervision and staff meetings.

  22. Attends and actively participates in all trainings and maintaining current certifications and licenses.

  23. Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  1. Bachelor’s or Associate’s in Nursing and must possess a current, state of MA, registration in nursing.

  2. Five years of experience in a community health care setting.

  3. One-year experience in supervising staff.

  4. Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.

  5. Demonstrate an interest in working with the brain injury population.

  6. Must be able to perform each essential duty satisfactorily.

  7. Must have sensitivity to the needs of the population.

  8. Strong computer knowledge.

  9. High energy level, superior interpersonal skills and ability to function in a team atmosphere.

  10. Excellent communication skills, written and verbal.

  11. Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
