Summary The Fayetteville VA Medical Center (VAMC) is seeking a skilled Licensed Practical Nurse/Licensed Vocational Nurse (LPN/LVN), GS-7, to perform complex nursing assignments. This role requires comprehensive practical nursing knowledge and the ability to exercise independent judgment under general supervision to enhance veterans' functional potential. Applicants must submit their most recent SF-50 for verification; AWARD or LWOP SF-50s will not be accepted. Responsibilities The Licensed Practical Nurse in the GS 7 grade functions under the supervision of an RN and/or MD but must successfully and consistently demonstrate the exercise of independent technical judgment without the need for readily available supervision. They must have a thorough working knowledge of the most complex or difficult practices and procedures. This LPN may also serve as leader or co-leader in a nursing unit or for a specifically identified group of patients. The LPN/LVN GS-7 Nurse is responsible for a range of specialized patient care activities, with duties including but not limited to: Patient Care: Utilize knowledge and skills to implement specialized care activities for veterans, making observations on their status to initiate interventions aimed at improving, restoring, or maintaining optimal health. Program Regulations: Demonstrate knowledge of nursing program regulations and objectives to ensure compliance and contribute effectively to the nursing program. Care Plan Development: Collaborate with Registered Nurses (RNs) and Medical Doctors (MDs) in the development and regular updating of patient care plans, ensuring alignment with veterans' and families' expectations and needs. Clinical Collaboration: Work in partnership with RNs and other Health Care Providers (HCPs) to promote and exemplify nursing concepts within the Medical Center. Medication Administration: Prepare and administer medications through various routes (oral, topical, subcutaneous, intramuscular, and intravenous) in accordance with established policies and monitor patients for changes, documenting and reporting any deviations. Communication and Intervention: Take proactive actions based on patient reports and observations. Communicate any significant changes in veterans' health to the appropriate healthcare providers, demonstrating the ability to intervene effectively. Assessment and Response: Utilize strong assessment skills to identify changes in veterans' health conditions, responding promptly to emergent situations and notifying relevant personnel. Reporting: Accurately document and report significant veteran information and collaborate with team members to modify care plans based on received data. Behavior Management: Use effective communication strategies to engage with veterans exhibiting challenging psychological and physical behaviors while implementing appropriate treatment modalities. Independent Functioning: Demonstrate the ability to work independently with minimal supervision while maintaining effective communication with veterans, families, and healthcare staff. Equipment Management: Perform treatments, procedures, and handling of reusable medical equipment relevant to assigned duties, ensuring proper reprocessing practices are followed. Participation in Quality Improvement: Actively engage in committees and groups focused on enhancing veteran care delivery and satisfaction. Sustainability Practices: Implement sustainable practices to reduce environmental impact and support VA's stewardship goals concerning natural resources. Supervision of Support Staff: Assist in monitoring the practices of unlicensed or non-medical staff to ensure safe delivery of patient care. Accountability: The LPN/LVN GS-7 Nurse reports directly to the Nurse Manager or designated supervisor and operates independently with guidance from an RN or HCP. Work is subject to general review for technical soundness and conformity to nursing standards. Additional Responsibilities: Participate in Home-Based Primary Care initiatives as needed. Maintain flexibility regarding scheduling and assignment adjustments based on unit needs. Conduct patient education and health promotion independently. Ensure compliance with safety protocols during the transport of medications and medical equipment. Physical and Other Requirements: Capable of performing physical tasks including moving patients and equipment, maintaining visual and auditory acuity, and managing potentially stressful situations. Complete annual Employee Health requirements, such as TB screenings. Valid state driver's license required for patient transport in GSA vehicles. Education: Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the Accreditation Commission for Education in Nursing (ACEN) at the time the program was completed by the applicant. Verification can be obtained from the State approving agency or the Accreditation Commission for Education in Nursing. Licensure: Full, active, current, and unrestricted licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia. English Language Proficiency: LPN/LVN appointed to direct patient care positions must be proficient in both spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-07 Completion of at least one year of additional qualifying experience at the GS-6 level or equivalent, fully meeting all the performance requirements for the GS-6 LPN/LVN and below.
