Job Title: Support Broker
Qualifications: Requires Minimum of BS in Human Services (Psychology, Sociology, Social Work, Special Education, or a field relevant to Human Services)
Hours: Monday through Friday 8:00 am until 4:30 pm. Other times may be required with the job.
This position reports directly to the PDS Director.
Summary of Job Duties: The Support Broker is the Case Manager for the Participant Directed Services within the 1915c waivers and may be Full or Part time with on call responsibilities. The participant is in charge of determining available services, scheduling, employing, budgeting and evaluating the usefulness of the services. They will train, provide technical assistance, answer questions, coordinate services and community resources, monitor service provision and assist in developing a person centered plan of care (POC) including as emergency back-up plan, support spending plan and budget. The Support Broker will ensure the participant’s health, safety and welfare by making face-to-face home visits, criminal background checks on potential PDS employees, and verifying Medicaid eligibility status.
Essential Duties and Responsibilities:
Include the following:
Working with an individual and others identified by the individual as their team to develop an Individual Plan of Care (POC) budget and spending plan, and emergency backup plan.
Utilize a person-centered planning approach to identify and implement all supports needed/wanted by the individual.
Maximum caseload of 40 PDS clients
Provide any needed assistance to a PDS participant with any aspect of PDS or blended services (i.e., recruiting/hiring/managing PDS providers, completing and submitting required forms.)
Coordinate all services.
Monitor a participant’s health, safety and welfare.
Provide a monthly face-to face home visit.
Be available 24 hours per day, 7 days per week, including holidays.
Document each contact with the participant or on behalf of the participant and the progress towards outcomes/goals
Resolve issues related to unmet consumer needs and/or health and safety are being jeopardized or funds in the consumer’s budget are not being utilized per the POC
Each staff person has a legal responsibility to report any form of abuse (physical, emotional, sexual), neglect or exploitation
Other duties as assigned
Required Job Skills:
Complete training requirements within specified time frames per SCL/MPW Waivers, DDID standards and procedures
Comply with agency policies and procedures, SCL/MPW regulations, DDID standards and procedures , JCAHO
Valid driver’s license and no moving violations on driving record
Clear Background Checks
Valid Social Security number or valid work permit if not a US Citizen
TB Risk Assessment Completed within 7 days of hire
Negative drug test
Read, adhere to and promote the MR/DD Vision Statement

Environmental Data and Job Hazards:
Hours of work are structured 7.5 or 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands.
Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials.
All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens.
All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned.
Must be certified in Cardiopulmonary Resuscitation with one month of employment and maintain the certification if providing direct care.
