APPLICANT WILL ASSESS THE STRENGTHS AND THERAPEUTIC NEEDS OF A CHILD AND FAMILY. ACTIVE LISTENING, ASKING QUESTIONS AND EXCHANGING INFORMATION WITH THE CHILD AND FAMILY ARE CRITICAL ELEMENTS OF THE ASSESSMENT, WHICH INCLUDES TRAUMA, DOMESTIC VIOLENCE, D&A, BMI, AND ALL OTHER DETAILS RELATED TO BEST PRACTICE. COMPLETION OF LETHALITY ASSESSMENT AND SAFETY/CRISIS PLAN AT INITIAL ASSESSMENT AND AT EACH SUBSEQUENT ASSESSMENT. WILL DETERMINE WITH THE FAMILY AND CASE MANAGER ANY NECESSARY FAMILY SUPPORT SERVICES. DEVELOPMENT WITH THE CASE MANAGER AND OTHER INVOLVED PROFESSIONALS OF A PLAN TO OBTAIN IDENTIFIED SERVICES. INCLUSION OF CHILD AND PARENTS OR OTHER CARETAKERS AS MEMBERS OF THE TREATMENT TEAM AND AS PARTNERS IN TREATMENT. SUCH INCLUSION REQUIRES THAT THE FAMILY ACTIVELY PARTICIPATE IN AN ONGOING MANNER IN THE FORMULATION, DEVELOPMENT, IMPLEMENTATION AND MONITORING OF TREATMENT EFFORTS. SUCH PARTICIPATION PRESUMES THE FAMILY'S BROAD KNOWLEDGE ABOUT THE CHILD AND THE FAMILY'S INTENTION TO CONTRIBUTE CONSTRUCTIVELY TO POSITIVE OUTCOMES. PARENTAL PARTICIPATION IN TREATMENT TEAM MEETINGS, TREATMENT PLAN DEVELOPMENT/REVIEW AND INDIVIDUAL EDUCATION PLANS (IEPs) IS ESSENTIAL AND SHOULD BE SUPPORTED BY THE MOBILE THERAPIST. INCLUSION OF SCHOOL REPRESENTATIVES AT ALL TREATMENT TEAM MEETINGS, WHETHER IN PERSON OR TELEPHONE). PROVISION OF CHILD-CENTERED, FAMILY FOCUSED INDIVIDUAL AND FAMILY PSYCHOTHERAPY, AS AGREE UPON BY THERAPIST AND FAMILY. IT IS EXPECTED THAT THE APPLICANT IN ADDRESSING DEFINED PROBLMS WILL UTILIZE STRENGTHS BASED APPROACH. SPECIFIC FORMATS FOR INDIVIDUALS AND FAMILY THERAPY VARY ACCORDING TO THE INDIVIDUALIZED NEEDS OF CHILD AND MAY INCLUDE IN THE HOME OR COMMUNITY SETTING SESSIONS WITH THE CHILD INDIVIDUALLY, THE ENTIRE NUCLEAR FAMILY, THE FAMILY AND COMMUNITY RESOURCE (MINISTER, SCOUTMASTER, COMMUNITY LEADER MENTOR, ETC), FAMILY AND TEACHER, GUIDANCE COUNSELOR OR PRINCIPAL, OR SUBSYSTEMS OF ANY OF THE ABOVE AS CLINICALLY INDICATED AND AGREED UPON. INITIAL TREATMENT PLAN TO BE DEVELOPED WITHIN 30 DAYS OF SERVICE INITIATION. TRAUMA IS ASSESSED AND ADDRESSED IN EVERY PLAN, AND TREATMENT PLAN REVIEWS OCCUR EVERY 6 MONTHS. APPLICANT WILL DETERMINE IN CONJUNCTION WITH CHILD AND FAMILY AND OTHER INVOLVED PROFESSIONALS OF THE CLINICAL NEED FOR SPECIAL EVALUATIONS AND SERVICES SUCH AS MEDICATION ASSESSMENT BY A PSYCHIATRIST, PSYCHOLOGICAL TESTING OR OTHER. COLLABORATION WITH CHILD AND FAMILY AND OTHER INVOLVED PROFESSIONALS TO DEVELOP DAILY ROUTINES DURING TIMES OF CRISIS AND TRANSISION, AS WELL AS A 24-HOUR CRISIS PLAN. WILL COLLABORATE WITH OTHER INVOLVED PROFESSIONALS AND AGENCIES IN OFFER TO PROVIDE UNIFIED SERVICES AND CONTINUITY OF CARE TO BOTH CHILD AND FAMILY. PROVIDE A MINIMUM OF 30% FAMILY WORK TO EACH YOUTH IN THE HOME, SCHOOL, AND/OR COMMUNITY. PROVISION OF A FACE TO FACE CLINICAL SESSION WITHIN 5 DAYS OF PROGRAM AUTHORIZATION AND A MINIMUM OF AT LEAST ONE CLINICAL SESSION PER MONTH. ACT AS A LIAISON AND CONSULTANT TO SCHOOL DISTRICT PERSONNEL. ATTEND ALL SCHEDULED SCHOOL MEETINGS (CORE, CHILD STUDY, SAP, ETC). PROVIDE ON-GOING WEEKLY SUPERVISION TO BHTS AND COMPLETE THE BHT PERFORMANCE CLIENT'S NEEDS ARE MET AND ADDRESSED. Minimum Requirement:  Applicant must have at least a Master's Degree in Social Work, Psychology, or Counseling, or Master's Degree in Nursing, but must have acerts by ANCC or AANP as a certified registered nurse practitioner.  Applicants must also pass FBI, State, and ACT 33 clearances.
