DESCRIPTION OF WORK In this position, you must have good interviewing and crisis intervention skills to conduct field visits to determine a level of care for the consumer. Field visits include community cases where consumers are living in their homes and facility cases where consumers are living in long-term care settings.
You must be able to prioritize their workday in order of importance and maintain a flexible schedule if you are called upon to respond to a consumer's emergency. This position will have knowledge of available community support services for the benefit of older adults and have a working relationship with consumers, outside agencies, the public, and other staff members. You will also assist with walk-in consumers for completion of forms, i.e., PACE, rebates, transportation applications, etc. Other duties include, but are not limited to, the completion of required statistical reports, travel vouchers, participation in conference calls, webinars, workshops, and mandatory trainings.
Work Schedule and Additional Information: Full-time employment Work hours are 8:00 AM to 4:00 PM, Monday - Friday, with a 30-minute lunch. Flexible schedule must be maintained to address consumer emergencies. Salary:In some cases, the starting salary may be non-negotiable. QUALIFICATIONS Minimum Experience and Training Requirements: Six months as an Aging Care Manager 1 or a County Caseworker 1; or Successful completion of the County Social Casework Intern program; or Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or An equivalent combination of experience and training.

Hiring preference for this vacancy may be given to candidates who live within Carbon, Columbia, Lackawanna, Luzerne, Monroe, Schuylkill, Sullivan, or Wyoming counties.
