What Success Looks Like In This Job

This position is designed for an experienced Community Support Case Manager who is highly skilled in the development, training, and accurate and timely issuance of all programmatic benefits. The successful candidate will work in both an independent and team-oriented atmosphere while utilizing strong behavioral and analytical skills to assist the QA/Training Supervisor(s) in development and performance analysis of all Community Support Case Manager team members, which includes: developing trainings, leading training cohorts, providing direction to team members and classroom management, performing case reviews, second level case reviews for accuracy, audit preparation, trend analysis, client merge activities, preparing case information for court hearings, and other duties list below. The candidate is expected to remain highly proficient with all current and future program policy requirements, as well as in the use of analysis tools, including Microsoft Excel, Access, and other databases, computer systems, and training methods. There are four primary areas of focus as a Community Support Lead, which includes training and development, certified trainer, quality assurance (QA), and claims.

Examples of Duties for Success

  • Complete case file reviews for Case Managers, identifying errors and providing feedback/training for correction as needed.

  • Complete Supervisor Authorizations in CBMS.

  • Validate accuracy of Lead and Supervisor Pre-Authorizations and Case Reviews (Third Party Reviews), identifying errors and providing feedback/training for correction as needed.

  • Pull case file records and prepare for State and Federal QA audits for all programs.

  • Complete data analysis of accuracy trends, root causes of untimely processing, and other key performance indicators (KPIs) as assigned by leadership, in a timely and accurate fashion.

  • Assist leadership in the identification of business practices and processes that impact the timeliness and accuracy of the department.

  • Assist with Management Evaluation preparation, coordination, corrective action plan development, and implementation of new business processes and practices.

  • Gather critical information and supporting evidence for corrective action plan or management decision letter rebuttals and submit to leadership or the respective State or Federal agency.

  • Coordinate, facilitate, and/ or participate in county workgroups designed to analyze root causes of identifiable QA trends, draft proposals to remedy, and submit to leadership for review.

  • Pull case files, accuracy and performance metrics, and case reviews as requested by State and Federal partners.

  • Assist with tracking performance benchmarks of new team members to assist with training requirements, trends, areas of success, and opportunities.

  • Assist with client complaints, research case and case managers’ actions and decisions, determine root causes, record complaint information, and remedy the client complaint in a timely and accurate manner.

  • Perform CBMS client merges as required.

  • Ability to work all program areas (like a Community Support Case Manager III) to assist the Community Support team during high work volume times.

  • Assist other Community Support Case Manager Lead focuses (Training and Development, Certified Trainer, and Claims) when needed.

  • Perform other duties as assigned, such as participating in CBMS User testing, staying current with CBMS Builds, participating in Knowledge Transfer calls and trainings, attending State eligibility program meetings, and supporting Economic Mobility Division training and timeliness efforts as needed.

  • Develop and deliver curriculum to meet key learning objectives and to strategically create connection points to the County’s goals and initiatives.

  • Utilize and train others to employ all research tools used in eligibility determination and apply those tools to information discovered through investigation and examination.

  • Ability To

  • Apply basic math and read, interpret, and apply manual regulations.

  • Communicate clearly and concisely, both verbally and in writing.

  • Work quickly and accurately, with minimal supervision at times, to complete time-sensitive tasks.

More Qualifications for Success

  • Experience:

  • At least three (3) years of experience determining eligibility for multiple programs.

  • Experience as a Community Support Case Manager II/III with consistently high case accuracy rate is preferred.

  • Working experience and knowledge of all programs – Adult Financial, Colorado Works, Food Assistance, and Medical Assistance. Experience with LTC is preferred but not required.

  • Education and Training;

  • High school diploma or GED equivalent required.

  • An Associate's or Bachelor's Degree is strongly preferred and may substitute for one year of required experience.

  • Additional analytical, statistical, or other related higher education is preferred.

  • License or Certificate:

  • None.

  • Background Check:

  • Must pass a criminal background check.

  • Ability to work in a standard office environment.
