Under the supervision of the Senior Vice President of Children Services, the Family Transition Specialist is responsible for providing transitional services to children in the Residential Program who are pending discharge, as well as to their families, to ensure success in the community upon the child's official discharge from the program. The Family Transition Specialist supports clients and their families for at least 6 months post discharge from Children's Residential Services. The position builds on treatment gains to promote the safety and well-being of children and families, with the goal of preserving the child in a supportive family environment. The position pays $20.00 to $24.50 per hour depending on experience.

DUTIES:
The CRCF Family Transition Specialist (FTS) is assigned to a youth's case thirty (30) days prior to the youth exiting the services of the provider.
Within seventy-two (72) hours of discharge, the FTS will have in person contact with the client and their family (including siblings as applicable), foster parent, or another placement provider.
Within the first seven (7) days following discharge, the CRCF nurse (RN or APRN-PMH-NP), will contact the member's parents or legal guardian to follow-up on any medical concerns or needs.
Assist the clinician with client and family treatment plans by entering progress notes from residential, day treatment , OT, and generalist into QUR forms and documenting client and family input from these meetings.
QUALIFICATIONS:
Bachelor's degree in Social Work, Psychology, or related discipline.
