Service & Support Administrator (Developmental Disabilities Case Manager)

SSA's work one-on-one with individuals with developmental disabilities and their families, both virtually and in-person. They carry a caseload for which they will coordinate services and supports using a person-centered approach, develop and revise individual plans and budgets, advocate for individual's choices and rights. Also, monitor service provision for quality, continuity, and effectiveness. Complete documentation guidelines, use designated computer software programs, communicate the availability of resources both within the agency and in the community while working cooperatively with providers and community agencies and attend and lead meetings as needed.

 

The successful candidate will be required to have a bachelor's degree in human services, special education, or other related field. Must be organized, accurate, self-motivated, able to work with others and have the ability to remain professional and compassionate in stressful situations.

This is a full-time 40 hour per week schedule requiring some flexibility, working from home and occasional on-call for emergency assistance. Pay is commensurate with experience and relevant education.
