Early Childhood Case Manager/Service Coordinator (SSA)

Qualifications: Successful candidates
- Must have a Bachelor's Degree in Human Services, Social Work, Special Education or another related field
- Must have or be eligible to obtain Ohio DODD SSA certification
- Must have or be eligible to obtain Ohio DODD Early Intervention Service Coordinator Certification.
- Must be highly self-motivated, self-disciplined, and able to work cooperatively with others
- Must have strong communication skills and leadership qualities
- Must be organized, accurate and have the ability to remain professional and compassionate in stressful situations.
- Must possess computer skills
- Must be willing to undergo criminal background checks, work reference checks and drug screening.
Preferred two years of experience with children with developmental disabilities
Job Summary: Successful applicants will
Work one-on-one with young children with developmental disabilities and their families, both virtually and in-person
- Participate in the eligibility process for young children
- Coordinate services and supports using a person-centered approach
- Lead the development of individual plans for children on caseload
- Communicate the availability of resources both within the agency and in the community
- Work cooperatively schools and community agencies
- Monitor service provision for quality, continuity, and effectiveness
- Complete required documentation, maintain confidentiality
Attend and lead meetings
Hours: Full-time 40 hour per week schedule requiring some flexibility, some ability to working from home
Salary & Benefits: Pay: Commensurate with experience and relevant education.
