Primary Function: The Targeted Case Manager is responsible for providing advocacy and service coordination services for children, adolescents, and their families in their home, at school and in their local community. The Case Manager also assists with navigating the policies and procedures of state agencies.

Essential Duties: These are the core component duties for the position and are not to be considered an inclusive list.

  • Assess and evaluate client needs, completing all necessary paperwork as required by state and MCH policies.
  • Be aware of and understand all local client resources.
  • Meet with clients and providers on a regular basis to maintain and coordinate services.
  • Make referrals to other agencies as needed.
  • Develop individualized service plans for all clients and update them on a quarterly basis or as needed.
  • Manage transition towards termination or referral to other services.
  • Respond appropriately to emergencies or unexpected situations.
  • Be aware of and adhere to the policies and procedures of Maine Childrens Home and the counseling center.
  • Participate in regular clinical supervision and seek additional supervision as necessary.
  • Using the electronic records system, complete clinical documentation within established timelines and professional standards per MCH guidelines, licensing/accreditation requirements, and those required by insurance or other state/federal funders such as Medicare and Medicaid.
  • Provide clients with information and education regarding their mental health needs, their rights, relevant area (and MCH) resources, and means of access.
  • Assure confidentiality and rights of clients.
  • Serve as an advocate for clients.
  • Participate in assigned quality assurance activities.
  • Meet or exceed the established billable expectations for the position.
  • Participate in community activities as schedules permit.
  • Participate with supervisor in creating an individual professional development plan that ensures compliance with licensing and supports the programs strategic goals.
  • Respond to requests from external agencies when appropriate and in a timely manner.
