Overview

The Role:\ The Permanent Supportive Housing Case Manager will provider case manager for the State\'s new Permanent Supportive Housing Medicaid Program. This includes assessment and eligibility determination, case planning, service authorization, and care coordination for participants. This position will be part of a small, dynamic team that partners with community-based service providers working to address homelessness in Vermont. Primary responsibilities include, but are not limited to:

Determines eligibility for program services; conducts a whole-person assessment of need with each participant and works with the participant to develop a person-centered care plan. Meet regularly with the participant to assess progress.

Coordinates services with local contracted providers; works collaboratively with the providers to ensure that all plans developed and all services delivered align with participants\' needs and priorities.

Refers to and collaborates with a range of providers including, but not limited to, Designated Agencies, Preferred Providers, Primary Care Physicians, and the VT Chronic Care Initiative.

Participates in quality assurance/quality management activities, including site visits, case reviews, and other monitoring activities.

You Bring:\ Commitment to housing first principles, trauma-informed care, and strengths-based relationships.

Knowledge of eligible health needs including mental health, substance use, complex physical health needs, delays in cognitive development, and those with assistance with one or more activities of daily living.

Knowledge of housing and homeless systems of care, evidence-based and emerging practices in human services and housing, and federal rules and regulations (Medicaid).

Ability to communicate effectively as well as establish and maintain effective working relationships.

Ability to perform job duties within the framework of the four key practices of the Agency of Human Services: customer service, holistic service, strengths-based relationships and results orientation.

Environmental Factors

Work is performed primarily in an office setting and in a variety of field situations. Private means of transportation should be available since travel to field sites will be required.
