Presbyterian Senior Care Personal Care Aides in WASHINGTON, Pennsylvania

Essential Duties and Responsibilities:

Include the following: Identifies and suggests innovative approaches and participates in implementing performance improvement opportunities. Maintains compliance with Presbyterian SeniorCare Network policy and procedure in accordance with governing laws, regulations, regulatory agencies, accreditation bodies and community standards. Demonstrates on a consistent basis individual and team behavior that supports the Person-Centered, Woodwell and Woodside philosophies of Presbyterian SeniorCare Network. Consistently upholds the mission, values and behavior standards of Presbyterian SeniorCare Network. Maintains confidentiality. Other duties may be assigned.

Provides assistance to residents with activities of daily living (ADLs) to ensure good hygiene, grooming and self-esteem. Promptly answers call lights.

Utilizes and assists in updating support plans in order to help the resident to achieve established goals and promote independence and quality of life. Attends care conference as requested.

Certification obtained by successful completion of DHS-approved Medication Administration training and/or approved insulin training by certified diabetic educator. Accurately assists residents with administration of medications. Accurately docuements medication administration in electronic health record (EHR).

Reports changes in resident's status through documentation and verbal communication to ensure continuity of care. Documents accurately in EHR to ensure good communication and accurate details of events.

Collaborates with other team/household members to ensure a person-centered culture in the meeting the needs and desires of residents and other staff.

Accompanies residents to and from various appointments and activities when necessary to ensure safety and timely arrival.

Conducts planned and spontaneous activities with residents to encourage socialization and promote self-esteem.

Assists in admission, discharges and transfers of residents to promote continuity of care. 

Serves food according to instructions to meet therapeutic needs. May prepare meals on some occasions. May monitor food and fluid consumption in order to identify dietary concerns.

Operates and maintains cleanliness of appliances and equipment to promote optimal efficiency and decrease unnecessary maintenance.

Completes all cleaning assignments to maintain quality standards.

Participates in all required training and education programs.

Understands and implements the principles of infection control by consistently using Standard Precautions in all resident contacts.

Is proficient in implementation of emergency procedures for accident, fire, weather alert, elopement or other disaster.

Education and/or Experience:

Must be a minimum of 18 years of age and have a high school diploma or GED OR must be 18 years of age and have documentation that the person in enrolled in and will receive his or her GED diploma or high school diploma within 6 months after hire.

Certificates, Licenses, Registrations:

To complete certification requirements in Dementia Care and DHS Direct Care training during orientation period.

Computer Skills:

Ability to use computer or laptop for documentation purposes.
