Job Overview:
The Mental Health Screener plays a critical role in providing prompt mental health assessments and crisis intervention services to individuals experiencing acute psychiatric distress. This position will assist in the facilitation of intake processes, collaborate with a multidisciplinary team, and ensure accurate documentation of all clinical activities to maintain program efficiency and quality patient care.
The S-Cope Program provides services to clients 55 and older in Long-Term Care Facilities, Assisted Living, and Hospitals settings, focusing on non-pharmacological interventions for managing behavioral issues.
Qualifications:
Required:
Master's Degree in Social Work, Counseling, Psychology, Special Education or other mental health related field
Minimum 2 years of experience in the mental health field, crisis intervention, behavioral health, or related services
Scheduling Requirements:
Shift working, Monday to Friday 8:30AM - 4:30PM, with additional support provided during on hours as needed
On-call responsibilities required, include coverage on a rotating basis: Monday to Friday 4:30PM - 8:30PM; Saturday and Sunday 8:30AM - 6:30PM

Essential Functions:
Designs and implements treatment plans, including individual, group, and/or family psychotherapy, and evaluates and documents patient outcomes according to department policy and procedure
Interprets the nature of illness and prescribed course of treatment for patients and families
Communicates family issues to other staff members
Participates in planning conferences
Maintains up-to-date and complete records of patient information according to established standards
As a member of the multidisciplinary team, assists in reviewing and updating treatment and discharge plans
Collaborates with other hospital programs and community agencies with which patients and families are involved
Assesses patients' needs through clinical interview, chart/history review and through use of standardized instruments
Performs age-specific assessments
Uses communications appropriate to age
Assesses age specific safety issues
Interprets patient data and responses
Provides an immediate response to all requests for service through the 800# and gathers initial information on the type of intervention that is needed
Makes further phone calls to obtain more detailed information from the referring agency, family, treating practitioners and others involved with the individual
Completes a written intake summary on the call and provides an intake summary to Advanced Nurse Practitioner (ANP) in that particular geographical area
Using crisis phone intervention skills, handles an individual in a psychiatric crisis and makes an appropriate clinical decision whether an emergency response is necessary
Makes follow-up phone calls on the intervention and determines if anything else is needed
