Job Summary:

Does marketing of MHG Sub-Acute Unit to acute and long-term acute facilities. Performs physical and financial assessments for patients referred from acute and long term acute. Completes and transmits MDS & TAR. Reports to Director of Nurses,

Essential Job Duties:

  • Assess and review, in collaboration with Pharmacy, Medical Director, Social Workers and DON, eligibility for prospective residents Inputs MDS data as an accurate representation of residents and transmits to the government according to regulations.

  • Knowledgeable and able to interpret/apply Medicare and Medical requirements with regard to MDS and TARs according to regulatory requirements Establishes and maintains a collaborative working relationship with the Interdisciplinary Team. Maintains confidentiality, security and integrity of departmental and resident information Serves as a role model for the Mission, Vision and Values of the organization and fulfills other job duties as requested within Scope of Practice Maintains current competencies as well as updated knowledge in specific area of practice.

Experience:

  • (3) Three years of nursing experience in a sub-acute or long-term care environment preferred.

Licensure/Certifications:

  • Current California LVN or RN license in good standing with California Board of nursing.

  • Current BLS for Health Care Provider
