Responsibilities

The Department of Pharmacy and Mayo Clinic Northwest Wisconsin (NWWI) Division of Community Cardiology, Department of Cardiovascular Medicine, Mayo Clinic and NW WI Primary Care Anticoagulation Clinic are seeking an Ambulatory Care Clinical Medication Management Services (MMS) Pharmacist to expand our pharmacist team in Cardiovascular Medicine. This is a full-time position working 8 a.m. to 5 p.m. Monday through Friday, based out of Eau Claire, WI. This role will be divided 0.7 FTE in Cardiovascular medicine & 0.3 FTE in Anticoagulation. The individual in this role will be responsible for MMS in patients with heart failure, dyslipidemia/statin intolerance, pulmonary hypertension, pre-antiarrhythmic admission visits, and global ASCVD risk. Within the cardiovascular practice there may be opportunity for remote work on occasion. The anticoagulation coverage will meet critical needs for both NWWI & Southwest Wisconsin remotely. As part of this role, the pharmacist will be responsible for direct patient care, care team questions, and team meetings, in addition to practice development, management, and expansion related to patient care activities. Additionally, the pharmacist will be an active member in quality improvement, research activities, and education.

This MMS pharmacist will work with various team members including physicians, advanced practice providers, nurses, and additional support personnel in assigned clinic areas. The MMS pharmacist will design, implement, monitor, evaluate, and modify patient pharmacotherapy to ensure that medication therapy is indicated, effective, safe, and convenient.

Job Specific Functions Include:

  • Completes documentation and billing processes for pharmacist activities.

  • Participate in collaborative practice and/or protocol-driven practice as appropriate.

  • Participates in quality improvement and resource management initiatives.

  • Participates in and supports research activities of the department and the clinic areas served.

  • Participates in precepting activities.

  • Supports the continuous improvement, patient safety, and various accreditation programs of the department and institution.

  • Provides support to other departmental functions and initiatives.

  • Collaboration with specialty pharmacies and insurance prior authorization teams.

Qualifications

Required Qualifications

  • Graduate of an ACPE-accredited Doctor of Pharmacy (Pharm.D.) program or Bachelor of Science (B.S.) with a minimum of 2 years relevant experience.

Preferred Qualifications

  • Qualified applicants must be licensed to practice Pharmacy in the states of Wisconsin and eligible for licensure in Minnesota, a graduate of an accredited College of Pharmacy, with a Doctor of Pharmacy degree or a Bachelor of Science degree.

  • If a candidate has a Bachelor of Science degree, a minimum of 10 years relevant experience is required.

  • If limited experience with Medication Management Ambulatory Care, completion of an accredited Medication Therapy Management (MTM) certificate program is required within 12 months of hire.

  • Successful candidates must be registered or eligible to be a registered provider of MMS/MTM through Wisconsin Medicaid (Forward Health) and Minnesota Health Care Program (Minnesota Medical Assistance).

  • Completion of an accredited PGY-2 pharmacy residency in ambulatory care or cardiology and experience providing care to patients with complex cardiovascular diseases, including pulmonary hypertension is preferred.

  • BCPS, BCACP, or BCCP certification is preferred.

  • Ideal candidates demonstrate excellence in teamwork, innovation, communication, & organization. Additionally, previous experience with diversity and inclusion, community service, committee involvement, research, quality improvement, and/or leadership involvement are desirable.

License or Certification

  • Current pharmacist license by the Board of Pharmacy in Wisconsin, or eligibility to obtain. Eligibility for licensure in Minnesota. May not independently fulfill responsibilities reserved only to a pharmacist until license is obtained.
