Position Description
The Department of Psychiatry and the Behavioral Sciences at the Keck School of Medicine of USC is seeking a California-licensed clinician to serve as the Clinical Case Manager. The case manager will work in both environments at the Psychiatry and Behavioral Health Services (PBHS) clinic and at Keck Medical Center of USC Healthcare Consultation Center II (HC2).
Job Duties
The Case Manager will:
Assist in the design of case management services and assessment for the PBHS and HC2.
Work with patients and others to obtain information to assess patient needs, recommend treatment, and assess treatment progress
Coordinate referrals to on-and-off campus services; provide regular follow up and assessment to ensure linkage and usage of referred services.
Establish treatment programs by setting schedules and routines, coordinating provided services, and arranging resources needed.
Provide risk assessment and crisis services, including the coordination and referral of 5150 assessments and hospitalizations and be the primary contact; assist with post-hospitalization coordination.
Assist with student leaves when working at PBHS and coordinate with campus partners, including providing documentation and support related to student leaves, as needed.
Provide regular check-ins with patients to promote continuity of care over holidays, summer, and while on waitlist for services
Coordinate client’s discharge by reviewing and preparing discharge plans and post-discharge requirements.
Monitor cases by verifying clients’ attendance and observing and evaluating treatments and responses.
Conduct urgent appointments or emergent walk-ins or intakes, particularly if patient’s clinician is unavailable.
Provide crisis intervention as needed
Maintain confidential client records by reviewing case notes and client progress, and logging events
Consult with service providers and community agencies on case management issues and needs
Coordinate with USC Student Health Care Coordinators, as well as Keck Medical providers and Keck Hospital social work team, to ensure that patients are connected with services
Communicate clients’ progress by participating in weekly interdisciplinary meetings and evaluations; disseminate results and obstacles to therapeutic team.
Complete timely billing and documentation of services provided.
Improve faculty and staff competence by providing educational resources.
Perform case management duties for assigned patients
May provide supervision of social work interns in Master’s degree programs in the future

Minimum Qualifications
Master’s degree in Social Work or Marriage and Family Therapy
Licensed in the state of California
Expertise in and prior experience providing case management and short-term treatment, including brief psychotherapy, stabilization and referral of serious mental illness, and crisis intervention
Resilient, self-aware, and can thrive in a fast paced, demanding environment
Multicultural competency in working with students and adults of color, from all economic classes, first generation students, LGBTQ identified individuals, and international students
Preferred Qualifications
Work experience in college counseling centers
Excellent diagnostic skills
Experience working with issues encountered in late adolescence, including: transgender identity, men’s issues, sexual trauma, eating disorders, and substance use disorders
Commitment to the highest ethical standards of professional practice and to professional integrity
Desire to continue to grow as a professional and committed to excellence

This is a faculty position in the Department of Psychiatry and the Behavioral Sciences at the Keck School of Medicine of USC. The position is full-time.
