Key Responsibilities:
Conduct initial and ongoing comprehensive patient assessments, including OASIS evaluations.
Collaborate with an interdisciplinary team to develop and implement personalized care plans.
Assist physicians in patient evaluations through muscle, nerve, and joint tests.
Educate and supervise patients and their families on therapeutic programs, as well as the use of wheelchairs, braces, crutches, and other mobility aids.
Provide and coordinate necessary equipment to support patient care plans.
Document clinical/progress notes within 48 hours of visits and communicate with physicians at least every 30 days or when changes occur.
Participate in case conferences, team meetings, and in-service training programs.
Assist in discharge planning.
Qualifications:
Successful completion of an accredited Physical Therapist Assistant education program.
Valid state licensed Physical Therapist Assistant.
Strong communication skills and ability to work independently.
Experience in home health care is preferred but not required.

The salary range for this position is $55,000-$60,000/year
