Guthrie Case Manager - Home Health NY in Vestal, New York
Guthrie Home Health New York (Vestal) is searching for a disciplined & compassionate Registered Nurse Case Manager in the Home Health setting in the Southern Tier region of NY. Our amazing team of Case Managers are critical thinkers that help provide a high standard of care to patients in their homes. One year of Med/Surg, ER or ICU type experience is required.
The hours are typically 8:00 AM - 4:30 PM but the position offers flexibility with accountability. You'll have flexibility to create your own schedule daily based on your patient load. The schedule is Monday - Friday with on-call and weekend rotation (typically 1 weekend a month).

Description:
Provides direct nursing care in accordance with established policies, procedures and protocols of the healthcare organization.
Education/License:
Diploma from an accredited school/college of nursing. Required professional licensure at time of hire.
Responsibilities:
Implements and monitors patient care plans. Monitors, records and communicates patient condition as appropriate. Performs direct participant care, including acute triage, blood draws, wound care, etc.|
Participates in the development, implementation, and evaluation of the Interdisciplinary Team (IDT) plan of care. Serves as a primary coordinator of all disciplines for well-coordinated patient care.|
Carries out physician and nursing orders. Attends team meetings and care plan meetings.|
Assesses and coordinates patients' discharge planning needs with members of the healthcare team. Participates, develops, and provides education to participants, caregivers, and associates.|
Participates in after hours/weekend on-call rotation, as needed. May require travel to participant place of residence, long term care and assisted living facilities.
