Are you a compassionate and skilled Registered Nurse with a passion for coordinating patient care? We're seeking a dedicated RN Transfer Coordinator to join our dynamic team at PeaceHealth. This is a Per Diem Variable shift position, perfect for individuals looking for flexibility and a rewarding career. Shifts are 12 hours and could include days, mids and nights.
What You'll Do:
As an RN Transfer Coordinator, you'll play a crucial role in ensuring seamless patient transitions. Your responsibilities will include:
Managing incoming direct admissions and patient transfer requests
Acting as a liaison between referring entities and PeaceHealth providers
Collecting and verifying patient information for accurate assessment and registration
Initiating protocols for critical care transfers and other high-priority situations
Utilizing advanced communication skills to coordinate with multiple stakeholders
Performing clinical reviews to ensure medical necessity and appropriate care
Contributing to quality improvement initiatives and maintaining regulatory compliance

What You'll Need:
Active RN license in the state of Washington or MSL.
Minimum of 3 years of acute care experience, including 1 year in ED/Triage/Critical Care/Charge.
Strong clinical judgment and problem-solving skills
Excellent communication and interpersonal skills
Proficiency in electronic health records and other relevant software
Ability to work independently and as part of a team
Flexibility and adaptability to changing priorities
