Are you a compassionate and skilled Registered Nurse with a passion for coordinating patient care? We're seeking a dedicated RN Transfer Coordinator to join our dynamic team at PeaceHealth. This is a Per Diem Variable shift position, perfect for individuals looking for flexibility and a rewarding career. Shifts are 12 hours and could include days, mids and nights.

What You'll Do:

As an RN Transfer Coordinator, you'll play a crucial role in ensuring seamless patient transitions. Your responsibilities will include:

  • Managing incoming direct admissions and patient transfer requests

  • Acting as a liaison between referring entities and PeaceHealth providers

  • Collecting and verifying patient information for accurate assessment and registration

  • Initiating protocols for critical care transfers and other high-priority situations

  • Utilizing advanced communication skills to coordinate with multiple stakeholders

  • Performing clinical reviews to ensure medical necessity and appropriate care

  • Contributing to quality improvement initiatives and maintaining regulatory compliance

What You'll Need:

  • Active RN license in the state of Washington or MSL.

  • Minimum of 3 years of acute care experience, including 1 year in ED/Triage/Critical Care/Charge.

  • Strong clinical judgment and problem-solving skills

  • Excellent communication and interpersonal skills

  • Proficiency in electronic health records and other relevant software

  • Ability to work independently and as part of a team

  • Flexibility and adaptability to changing priorities
