Children's Clinics Behavioral Health Care Coordinator in Tucson, Arizona

Children’s Clinics is seeking a Full-Time Behavioral Health Care Coordinator!

This position is responsible for:

  • Conducts a comprehensive psychosocial assessment, summarizes primary presenting issues, formulates diagnosis, and makes treatment recommendations. Utilizes best practices (motivational interviewing, trauma informed approach, strength-based approach)

  • Conducts annual assessments, administers various screening tools and develops individual service plans

  • Provides crisis assessment and provides crisis intervention

  • Provides case management services to members and their family and refers members/family to appropriate community resources as necessary

  • Coordinates services, develops and monitors individualized service plans

  • Performs other duties as required and assigned

Qualifications

MINIMUM QUALIFICATIONS

Education:

  • Bachelor's degree in the field of social work, psychology, or related subject area

  • An Associate's Degree or HS Diploma may be considered with relevant years of behavioral health experience that comply with Office of Behavioral Health Services staff qualification requirements

Experience:

  • One or more year’s experience with case management required

  • Experience with children, youth, and families in a behavioral health setting preferred

  • Bilingual English/Spanish speaking required

Licenses and Certificates:

  • BLS certification required

PREFERRED QUALIFICATIONS

Knowledge, Skills and Abilities:

  • Skill in conducting and documenting patient assessments

  • Skill in intervention and collaboration

  • Skill in problem solving, critical thinking, and detailed organization

  • Ability to communicate effectively, both verbally and in writing with patients, their families, clinic staff and community members

  • Ability to operate personal computer and related office equipment as needed to perform job function

Fingerprint Clearance:

  • Employees are required to maintain a current Department of Public Safety Level 1 Fingerprint Clearance Card.

  • Proof of a current fingerprint clearance card must be received within 90 days of employment.

While performing the duties of this job, the employee may potentially be exposed to infectious organisms during routine and emergency situations. The noise level in the work environment is usually moderate.
