Unity House Clinical Supervisor CTI (Critical Time Intervention) Team Leader in Troy, New York

Come be a part of our newest Clinical Program, the Critical Time Intervention (CTI) multi-disciplinary team.
We are looking for an innovative Team Leader who is a NYS Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker (LCSW).
CTI is a time limited, evidence-based care management approach focused on continuity of care for individuals with severe mental illness during times of transition from an in-patient setting back into the community. The Team Leader will lead a unique project that partners Unity of House of Troy in Rensselaer County and Mohawk Opportunities in Schenectady County together to provide a multi-disciplinary team approach to supporting participants. The Team Leader will provide oversight and support to the multi-disciplinary team and supervise and coordinate care for participants. Position is based at Unity House of Troy in Rensselaer County, but requires frequent travel to and from Mohawk Opportunities in Schenectady county as well as other travel in the community.
This is a full-time, exempt position that does not have any on-call responsibilities.
Responsibilities:
Lead the multidisciplinary Critical Time Intervention (CTI) team.
Supervise all professional and paraprofessional staff, ensuring demonstration of core competencies initially and in continued job performance.
Provide administrative oversight to the team.
Manage challenging situations while prioritizing competing demands.
Ensure this evidence-based practice is delivered by a mobile, multi-disciplinary mental health treatment team.
Coordinate a full range of clinical treatment, psychosocial rehabilitation, and community support services to help to promote recovery by improving psychiatric symptoms.
Work with local hospitals on participant discharge and discharge planning.
Deliver services in a community-based setting including home visits and teaching skills to support participant success.
Effectively collaborate with staff across the agency and also shift roles from Team Leader to direct service delivery.
Create and promote a culture of inclusion and belonging.
Requirements:
Must be a NYS Licensed Mental Health Counselor (LMHC) or Licensed Clinical Social Worker. Please have 3-5 years post graduate experience working with individuals with mental illness. At least 3 years prior experience in a supervisory role, preferably leading a multi-disciplinary team of individuals. Valid and Clean New York State driver license is required. Reliable transportation. Ability to work as a team and independently is a must.
