Position Overview Under the supervision of the Manager of Team Care Coordination the Returning Citizen Community Reentry Program Case Manager provides direct case management services to homeless individuals, especially individuals in the re-entry population, with a history of criminal justice involvement. The Case Manager works towards the goal of moving clients quickly to an appropriate housing opportunity while addressing their specific needs, including substance abuse treatment, mental healthcare, physical healthcare, identification, and income development. Qualifications For Position Education: Bachelor’s degree required, Master’s in Social Work or related field preferred Experience: Minimum of three (3) years experience in case management. Preference for prior experience with re-entry population and with homeless individuals. Skills and Abilities: Active learning and listening, critical thinking, monitoring, reading comprehension, written comprehension and expression, and understanding of addictions Certifications and Licenses: Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) preferred Machine, Tools and Equipment Operated: Computer, Microsoft Office (word and excel), Homeless Information Management System (HMIS) Other Requirements: Valid driver’s license Job Specific Duties & Responsibilities Case Management
Provides direct case management services and support to individuals and clients, such as managing referrals and resolving issues
Collaborates with key partners, including financial, housing, substance abuse treatment, and criminal justice agencies, in accordance with Rescue Mission policies, procedures, guidelines ad applicable state regulations
Coordinates and provides direct counseling and supportive services to assigned clients.
Participates in a team approach to treatment including coordinating client services with other Rescue Mission staff and other provider agencies
Assists in securing housing, treatment plans and other referrals in support of client treatment plan
Encourages client’s participation in recovery support programs
Conduct assessments for individuals with a history of criminal just involvement utilizing the shelter who need substance abuse treatment services
Develops, implements, reviews, revises treatment plans under supervision of Team Care Coordinator
Facilitates the processing of client referrals and placements
Monitor client outcomes
Documentation
Initiates timely documentation of services delivered
Prepares and maintains client charts, roster, records, and reports
Reviews charts to ensure all necessary documentation is complete
Documents according to all licensure standards and established policies and procedures
Community Relations
Develops relations with other providers and referral agencies
Understands, researches and analyzes community needs
Uses analysis to provide program direction and goal recommendations
Advocates on client behalf, policy, and program level
Informs supervisor of industry trends and education needs
Safety
Tours physical facility to ensure safe environment for staff, clients, residents, and visitors
Maintains clean and orderly work areas
Assists with evacuations during drills and emergencies
Notifies supervisors of facility equipment issues
Miscellaneous
Participates in staff meetings
Participates in the determination of organizational policies regarding eligibility, program requirements, and program benefits
Communicates program trends to supervisor
Participates in ongoing training and education to maintain or obtain licensures/certifications
Performs other duties as assigned
Annual Salary:$44,800Schedule:Tuesday and Thursday 12:00pm to 8:00pm; Wednesday, Friday and Saturday 9:00am to 5:00pm

Covid-19
Rescue Mission of Trenton requires all new employees to provide proof that they are fully vaccinated and have received a booster (where eligible) against COVID-19 prior to start of employment, unless the Rescue Mission of Trenton has granted the employee a medical or religious exemption. Employees who are not eligible for a booster at the time of an offer of employment provide proof they have received a booster upon eligibility and upload proof of same. Failure to provide proof of primary vaccination and booster will result in recession of a candidate’s offer of employment or disciplinary action up to and including termination.
