Ministry Support (Diakon Lutheran Social Ministries) HUD Service Coordinator in Topton, Pennsylvania
Category/Shift/Hours per 2 week pay period: Full-Time / Day Shift / 75 scheduled hours per pay period

General Responsibilities:
- Responsible for helping residents obtain supportive services from the community that are needed to enable independent living and aging in place.
- Refers and links the residents to supportive services provided by the Community such as, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy.
- Provides case management when service providers are not available through the general community.
Creates a directory of providers for use by both development staff and residents.
Education/Training: A Bachelor's degree in Social Work, Gerontology, Psychology, Counseling or related field of study normally required.
Experience: A minimum of three (3) years' related experience normally required.
ADA: Ability to ability to bend, stoop, sit, reach, and lift items weighing up to 25 lbs.; visual and auditory skills.
