The role you’ll contribute:

The Patient Logistics Operations Coordinator is a professional and empathetic individual who not only manages the flow within the organization but is also knowledgeable about all aspects of the patient's admission and transfer process. They serve as the hospital liaison for patients and families, supporting the Patient Logistics Command Center, throughput activities and intake process to ensure timely and efficient placement. The coordinator actively participates in providing outstanding customer service and takes responsibility for maintaining relationships that are equally respectful to all.

This position requires an adept multitasker who can quickly assess and prioritize projects based on deadlines, complexity, process flow and hours required. The individual must possess active listening skills, problem- solving abilities, provide effective solutions, and be a team player and motivator. The person in this role manages multiple priorities in a fast-paced environment with frequent interruptions and high phone activity.

This individual is responsible for planning, prioritizing, organizing, directing, and controlling all admissions and transfers between nursing units. They also assist with the intake process for the Patient Logistics Divisional Transfer Center.

The value you’ll bring to the team:

  • Manages, directs, or coordinates patient logistics functions/services, to include but not limited to: ER Admissions, Surgical Services/PACU Placement, Internal patient movement, Direct Admits (Excellence)

  • Supports all of WFD patient logistics movement from all 11 hospitals and 5 free standing OSEDs.

  • Initiates off-side ED transportation dispatch and care coordination (Excellence)

  • Supports PLCC WFD Transfer Center in emergency intake coordination and completing callouts to admit and for additional consults. (Teamwork)

Qualifications

The expertise and experiences you’ll need to succeed :

· LPN or Paramedic

· Licensed Practical Nurse or Completion of a paramedic program.

· Basic Life Support (BLS) certification
