JOB PURPOSE:

Assists the Social Services Director with the day-to-day activities of social services within the building assigned. In accordance with the company policy and all applicable State and Federal laws, regulations and professional guidelines.

KEY RESPONSIBILITIES:

  1. Assists patients to dining area for meals; assists in feeding of patients as needed; passes food trays to individual patients in accordance with diet cards on tray and needs; documents intake of nourishment. Is able to identify the four basic food groups on a sample breakfast, lunch, and dinner meal

  2. Assists with admissions, discharges, and transfers as requested; understands and utilizes care plans

  3. Collects stool and urine samples per care plan; cleans bedpans and empty catheter bags, provides external/indwelling catheter care

  4. Displays proper use and care of lift equipment

  5. Offers fluids at appropriate times (including routine ice and water rounds) and distributes fresh water to patients and picks up used water pitchers.

  6. Properly documents in accordance with established guidelines; receives and gives reports on patient’s status or condition.

  7. Provides for bowel and bladder training/retraining, provides colostomy care

  8. Provides range of motion exercises for patients

  9. Provides routine care for the bedfast patient; turns patient intermittently if patient is bedridden due to illness

  10. Recognizes and reports signs/ symptoms of abuse and/or change in condition

  11. Removes bedding and replaces with clean linens per schedule or condition

  12. Takes vital signs of patient, e.g., temperature, blood pressure, and respiration and records information and documents/reports information regarding the patient’s health, I&O, any change of medical condition, and care given

  13. Possesses and demonstrates working knowledge and ability to apply standards of professional social work practice in long term care setting.

  14. Develops and maintains social work policies and procedures reflecting current professional standards and philosophy of care consistent with State and Federal regulations.

  15. Communicates directly with residents, family, staff, interdisciplinary team members and Department Heads to coordinate care and services, improve organization and implementation of plans of care to maintain a high quality of care.

  16. Demonstrates knowledge and respect for the rights, dignity and individuality of each resident in all interactions.

  17. Demonstrates competency in the protection and promotion of and advocacy for residents’ rights.

  18. Possesses thorough knowledge and understanding of quality improvement processes as well as emergency and safety requirements.

  19. Applies & releases restraints, and provides exercise

  20. Assists patients in and out of bed and into wheelchair with the correct use of the transfer belt and practices proper body mechanics while moving/transferring patients

  21. Assists patients in bathing: to include bed baths, tub baths, and showers

  22. Assists patients in dressing or undressing, and personal grooming e.g., oral/denture care, brushing hair, trimming fingernails and toenails, skin care and shaving

  23. Responds to resident behavioral and psychiatric issues by completing behavioral and psychosocial assessments, providing treatment recommendations and making referrals to appropriate mental and behavioral health providers.

  24. When necessary or assigned, maintains a social services caseload; providing social services to residents as required.

  25. Consult on a regular basis with the facility’s Behavior Management Program to ensure that the program is properly implemented and functioning to meet the psychosocial needs of the resident.

  26. Able to understand and follow written and verbal directions.

  27. Demonstrates honesty and integrity at all times.

  28. Demonstrates knowledge of emergency and disaster protocols.

  29. Demonstrates ability to prioritize responsibilities and complete projects within the allotted time and within the allotted budget.

  30. Able to respond to change productively and to handle additional projects as assigned.

  31. Maintain skills and knowledge through continuing education Applies information to job responsibilities.

MINIMUM EDUCATION REQUIRED:

High School diploma or equivalent.

MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:

Active, current and unrestricted Certified Nursing Assistance certification in the appropriate state

Should be a certified nurse assistant in accordance with the laws of the issuing state

MINIMUM EXPERIENCE REQUIRED:

One (1) year experience in social work.

ADDITIONAL QUALIFICATIONS: (Preferred qualifications)

A degree in Social Work or related field is preferred.
