SUNY Upstate Medical University Hospital At Home Advanced Practice Provider in Syracuse, New York

Job Summary:

Upstate is in search of an Advanced Practice Provider to provide care and oversight for patients within the Hospital at Home (HAH) program, which includes Telemedicine visits as well as Inpatient rounding at the Downtown and Community Campus, providing education and interface with primary teams, patients and families prior to transfer to the home setting.

Responsibilities will include but not limited to:

  • Significant interdisciplinary collaboration with Upstate's Team.

  • Provide expert level clinical care

  • Coordination of care via a full range of services that will improve quality of patient care

  • Assessing & evaluating patients, ordering appropriate laboratory testing & diagnostic studies.

  • Arranging for referrals, testing, consultation & therapeutic services, and

  • Maintaining outstanding relationships with families, referring physicians & all members of the health care team.

As an APP you are a member of the multidisciplinary healthcare team which demonstrates professionalism and leadership within the institution, exemplifying the Upstate mission, vision and values and acts in accordance with Upstate policies and procedures.

Minimum Qualifications:

Master’s Degree as a Nurse Practitioner or Physician Assistant.

NYS Licensed/Registered NP or PA.

Current National Certification consistent to the clinical practice.

NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire.

CPR-BLS Certification required at the time of appointment.

Must successfully pass pre-employment and credentialing clearances.

Preferred Qualifications:

1-2 years APP experience in Medicine.

Acute Care preferred.

Work Days:

Monday - Friday, Days, with possible weekend and holiday rotation.
