Summary Nurse-Family Partnership Program is a home visiting program. Registered Nurses (BSN) are responsible for providing comprehensive nursing services to first-time mothers and their families eligible for the Nurse-Family Partnership (NFP) program. Nurse Home Educators (NHE) are pivotal in transforming the lives of venerable babies, mothers and families by implementing and maintaining the fidelity of the NFP model. NHE educate, support and encourage new mothers to advocate for themselves.
Job Duties Assesses physical, emotional, social, and environmental needs of women and families and utilizes reflective processes to improve practice. Consults and collaborates with other professionals involved in providing services to women and families. Assists women and their families in establishing goals and outcomes. Participates in the reflective process in weekly case conferences, team meetings, and 1:1 clinical supervision meetings. Formulates nursing diagnosis based on nursing assessment and client goals; evaluates progress periodically. Provides education, support, and referral resources in assisting women and their families in attaining targeted goals. Adheres to standard policies regarding infection control, fire and safety, hazardous chemicals, personnel policies, and departmental procedures. Applies home visitation skills such as learning about community and community systems/resources. Participates in review and analysis of reports for achievements and areas for improvement. Pursues knowledge in maternal-child health and nurse home visitation that reflects most current nursing practice. Participates in continuous quality improvement efforts.
Minimum Qualifications Bachelor's Degree in nursing 2 years of experience in maternal/child health, public health, home visiting, or mental/behavioral nursing. Ability to complete duties under stress, deadlines, and while attending to multiple tasks simultaneously. Basic computer skills. Demonstrates palliative qualities and characteristics such as dependability, empathy, trust, respect, professional intimacy, and awareness of power differentials. Physical Demands Lift and carry 25 lbs. frequent sitting/standi g, frequent keyboard use, *patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
