Hospice Of San Joaquin RN Case Manager in Stockton, California

POSITION PURPOSE/SUMMARY: Provides skilled nursing assessments, planning, and care in compliance with the interdisciplinary team plan of care, under the supervision of the Director of Palliative Care and the direction of the physician according to the policies and procedures of Hospice of San Joaquin and the Palliative Care program.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES: 1. Provides Palliative education/information, emotional support, and community resource information to program participants who have to advance chronic or life-limiting illness via telephone communication and/or perform home/facility visits. 2. Using clinical assessment skills completes the initial nursing assessment; and identifies the physical, emotional, physiological, spiritual, and practical needs of the patient and family. 3. Develops treatment plans based on identified needs, plans reflect a problem-solving approach; defined outcomes are measurable 4. Utilizes Hospice of San Joaquin electronic medical records systems for patient documentation, and maintains up-to-date patient records. 5. Maintains up-to-date patient records so that problems, plans, actions, and goals are accurately and clearly stated and kept in accordance with Medicare, medical, State home health agency, and JCAHO standards. 6. On an ongoing basis, use clinical assessment skills specific to the Palliative Care patient population to update the plan of care and to give suggestions to the IDT for action and follow-up by various team members. 7. Interventions reflect an advanced level of nursing practice and commitment to the execution of the IDT plan of care. 8. Ensures HIPPA compliance. 9. Provide referral outcome information to the managing physician. 10. Provide educational and informational visits to referral sources/agencies. 11. Maintains a professional appearance of self and work area; exhibits appropriate behavior with customers/participants and staff, i.e. respectful, courteous, supportive, and professional at all times. 12. Uses assessment skills to identify the physical, emotional, spiritual, physiological, and practical needs of patients and families. 13. Provides telephone support or visit based on the identified needs, interventions reflect a problem-solving approach and measurable outcome. 14. Interventions reflect an advanced level of nursing practice and commitment to the execution of the IDT plan of care. 15. Uses expert knowledge of hospice nursing, crisis intervention, and family dynamics to determine the appropriate level of care. 16. Uses expert nursing judgment to function as a patient advocate, especially when seeking any change in the level of care for optimal, appropriate, and safe caregiving. 17. Collaborates efforts with pharmacy, DME, and other various health care providers (lab, MD, hospital, etc.) and members of IDT to provide appropriate care. 18. When necessary, coordinate care with skilled nursing facilities or acute care hospitals.  19. Functions as an on-call nurse, and utilizes expert knowledge of Palliative Care nursing, home health care, crisis intervention, and family dynamics to determine necessary for home visits. 20. Follows universal precautions in delivering patient care. 21. Demonstrates creativity in meeting patient/family needs in the home environment. 22. Supervises and documents the Home Health Aide plan of care. 23. Additional Duties as assigned by the Director of Palliative Care. 24. Follows policies and procedures of Hospice of San Joaquin and Palliative Care Program.

 

QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily and be compliant with respect to nondiscrimination under the Hospice of San Joaquin Personnel Policy and Procedures Handbook.                  EDUCATION and EXPERIENCE Graduate of an accredited school of nursing in the State of California as a Registered Nurse. Three years of recent clinical experience. Computer skills. Ability to learn and assimilate the clinical computer program system. Knowledge of advancing chronic or life-limiting illness/conditions, signs/symptoms, treatment interventions, and medication for palliative care. Ability to handle multiple projects and tasks simultaneously; uses time and resources efficiently to accomplish work objectives. Ability to work in a high-stress environment or with participants who are under stress and maintain a positive work attitude while working with customers/participants, staff, and the community. LANGUAGE SKILLS Excellent verbal and written communication skills. Ability to analyze, and interpret general business periodicals, professional journals, technical procedures, or government regulations.  Ability to write reports. Ability to effectively present information and respond to questions from groups or managers, clients, customers, and the general public. Demonstrated ability to write in a clear, understandable, and concise manner.

MATHEMATICAL SKILLS Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percent.

REASONING ABILITY Ability to apply common sense understanding to carry out instructions furnished in written or oral form. Ability to deal with problems involving several concrete variables in standardized situations.

CERTIFICATES, LICENSES, REGISTRATIONS Possess reliable transportation and have a valid California driver’s license and have automobile insurance which meets State standards. All employees who drive need to provide Hospice of San Joaquin with evidence of insurance. Possess a license to practice as a registered nurse in the State of California.
