Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Position Overview :
The Program Manager (GUIDE Model) is responsible for implementation of the GUIDE Model in all markets while working collaboratively with the Director to ensure alignment with company strategy and forecast. The GUIDE Program Manager is responsible for the effective implementation and day-to-day operations of the GUIDE Model across assigned markets. The Program Manager oversees the Care Navigators throughout the organization, providing support and guidance to ensure proper care delivery.
This position will establish the tools, processes, and systems to optimize the role of the Care Navigator. The Program Manager is expected to work effectively across departments and collaborate with colleagues to carry out day-to-day operations. She/He will aim to develop talent within the team, including acquiring, engaging and retaining Care Navigator staff.
Schedule: Full-time position. Hours (with some flexibility on start/end times) are 8am to 5pm Monday thru Thursday & 8am to 3pm on Fridays.
Location: Up to 25% local travel is required for this position. The remaining time is spent working remote (work from home).
Salary: $80,000 - $90,000. Base salary will be commensurate with experience.

Responsibilities :
Operational Management
Oversee the day-to-day implementation of the GUIDE Model in assigned markets
Monitor program operations and recommend adjustments to improve performance and efficiency
Ensure all program activities comply with organizational policies and regulatory requirements
Collaborate with senior leadership to achieve the growth, revenue and profitability targets for the GUIDE Model
Monitors team operational and financial performance
Team Leadership and Support
Organize and assist in the interviewing, training and educating of Care Navigators
Provide supervision and ongoing support/mentorship to Care Navigators
Provide coaching, corrective action, and disciplinary action to direct reports
Grow and maintain partnerships with communities and stakeholders involved in GUIDE Model delivery
Process and Technology Optimization
System development participation (ie: training, education, efficiencies, process improvement, etc.)
Policy and Procedure development
Design and develop infrastructure around people, process, and technology to achieve scale working in a matrix environment
Ensure compliance requirements are incorporated into standard operating procedures
Is an expert in Bluestone technology (EMR, Bluestone Bridge, etc.) and communication platforms and hardware (mobile devices and apps, software), and assists others on the team to develop competence when communicating with internal and external stakeholders and using equipment to update electronic medical records and other databases as appropriate
Qualifications :
Education/Certification/Experience
Licensed Social Worker or Bachelor level RN
Five (5) years experience in healthcare organization required
Five (5) or more years of progressive experience in management roles
Customer service and Quality related positions a plus
Knowledge and experience with Assisted Living and Memory Care communities a plus
Must have a valid driver’s license and minimum state required liability auto insurance
