We are seeking FT Overnight (After Hours) RN Salary: $76,500-93,500 annually, depending on experience Service Area: Twin Cities Greater Metro. Plus mileage reimbursement of .67 cents per mile. The After Hours RN is responsible to perform home care services and is experienced in nursing, with emphasis on end of life education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. The After Hours RN is adept at symptom management and brings peace to crisis situations. Position Functions & Responsibilities: Answers calls from client and family to determine hospice PRN needs. Provides a complete physical assessment and history of current and previous illness(es) Provides weekend/evening follow-up on client nursing needs Follows the care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and palliative nursing actions. Includes the client and the family in the planning process Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician. Provides direct client care as defined in the State Nurse Practice Act Counsels the client and family in meeting nursing and related needs Provides health care instructions to the client as appropriate per assessment and plan of care Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the client Lead by example in establishing rapport with clients, clinicians, partners, and the community Ensure positive/professional communication within the team Customer service resolution; escalate concerns Prepares clinical notes and updates the primary physician when necessary with change in condition Communicates with the physician regarding the client's needs and reports any changes in the client's condition; obtains/receives physician's orders as required Communicates with community health related persons to coordinate the care plan Timely completion of all documentation as it relates to the care plan by 10am the next day Understands and complies with all the rules and regulations that govern the responsibilities of a registered nurse Completes orientation and annual training requirements as assigned Participates in Lifespark's Event Management process including initiating repo ts, escalation of events and investigations Promotes safety in the workplace through real time coaching and participation in safety awareness training Manages client supplies assuring appropriate DME provider is ordered and utilized Orders new medications as determined by Hospice Medical Director, and reorders medication for urgent refill needs Schedules and keeps member appointments Organizes travel efficiently and documents mileage accurately within EMR Other duties as assigned Qualifications One to two years of recent LTC experience in an ALF/MC/SNF setting. One year of hospice care experience preferred Experience and desire to perform direct patient care necessary Graduate of an accredited school of nursing Current Minnesota Nursing License On-call position: every other weekend 5pm Friday until 8am Monday, as well as 2-3 weeknights 5p-8a Participates in holiday rotation Strong phone communication skills Proficient with technology and technological applications: Microsoft Office, Smart Phone, Calendaring, Wi-Fi, EMR, Internet Browsing, Printer Contact Lifespark Now!
