The After Hours RN is primarily responsible to plan, organize and direct home care services and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities. Position Functions and Responsibilities: Completes assessments of patient and family to determine skilled home care needs. Uses health assessment data to determine nursing diagnosis. Assess the problem based on conversation with client/patient, family or caregiver Uses clinical judgment, knowledge and experience to direct conversation and guide decision-making to give self-care advice, refer client/patient to physician, instruct client/patient contact emergency assistance, or dispatch appropriate community resource. Provides direct/triage client/patient care as defined in the State Nurse Practice Act. Counsels the client/patient and family in meeting nursing and related needs. Provides health care instructions to the client/patient as appropriate per assessment and plan of care. Provides education when appropriate. Reassures the rapid and effective transmission of client/patient data between all relevant parties. Promotes evidence-based practice. Establishes and maintains strong collaborative arrangements with other health care professionals. Completes skilled visits to prevent rehospitalizations. Completes annual mandatory training. Completes evaluation visits and routine visits. Lead by example in establishing rapport with client/patient. Ensure positive/professional communication within the care team. Customer service resolution; escalate concerns. Completes clinical documentation same day and updates primary physician when necessary. Communicates with physician regarding the client/patient needs and reports any changes in the client/patient condition; obtains/receives physician's orders as required. Communicates with community health related persons to coordinate the care plan. Extremely skilled at listening and interpreting subtle indications such as tone of voice, hesitations or incomplete responses. Inspires others and encourage them to seek advice and solutions to problems. Honors Client Confidentiality, Rights, Privacy, and Reporting Maltreatment Expectati ns. Adheres to Emergency Procedure and Response Expectations. Identifies home safety issues and notifies appropriate community resource. Demonstrates proper procedures: handling, lifting, transfers. Escalates employee work injury per protocol. Overview of Process, Types of Reports and Forms Follow-up Required - Escalating Event Other duties as assigned Manage safety concerns for clients and clinicians, escalating when appropriate Qualifications: One to two years of recent acute care experience in an institutional setting One year of home health care experience preferred Recent triage experience in clinic, ALF, SNF, or Home Health Experience in customer service/direct member contact field preferred Experience supervising caregivers Graduate of an accredited school of nursing Current Minnesota Nursing License Must pass a MVR check based on Lifespark company policy Contact Lifespark Now!
