Job Title: Remote Case Manager RN

Job Description

As a Remote Case Manager, you will be tasked with reaching out to Medicare members to enroll them in the Care Management program. Your responsibilities will include building care plans, completing medication reviews, and conducting care plan assessments. You will complete 20 calls per day, evaluate members' needs, and facilitate care plans for optimal outcomes. Additionally, you will develop ongoing care plans, collaborate with providers to identify specialists or community resources, and coordinate between members, their families/caregivers, and the care provider team to ensure adequate service provision. Your role also involves providing ongoing follow-up on member status changes and progress towards care plans, offering resource support such as employment and housing assistance, and educating members and their caregivers on disease processes, care options, and healthcare benefits.

Hard Skills

  • RN Compact License

  • Prefer RN License MI, OR or CA in addition to compact

  • Medicare experience

  • Home healthcare/Hospice background

  • Remote Case Management experience is preferred

  • Utilization Management software (Trucare preferred)
