Baystate Health Nurse Coordinator Total Joint Program Pre-Admission Testing Clinic in Springfield, Massachusetts

Summary:

The total joint program nurse coordinator is responsible for the development, coordination and maintenance of the care continuum for total joint patients. The total joint nurse coordinator provides leadership for all aspects of care beginning before surgery through one year postop; including pre-admission evaluation, peri-op services, inpatient units, post acute care and all other applicable entities within the orthopedic service line related to evidence-based practice of orthopedic patients. The total joint nurse coordinator coordinates and oversees the ongoing disease-specific certification process to continually improve care for the orthopedic patient population. Reports to nurse manager or service line director. Bachelors required, masters preferred

Job Responsibilities:

1) Partners with identified medical leaders to develop and implement expert clinical care delivery systems for a defined patient population or service.

2) Demonstrates outstanding clinical and service delivery and utilizes critical thinking, problem solving and expert knowledge to identify and facilitate systems improvements with a strong focus on health promotion, risk reduction and population-based health care.

3) Assumes leadership role in planning and implementing clinical initiatives and quality improvements that demonstrate effective positive progress in key performance metrics. Participates in data collection and analysis of clinical outcomes of care, customer satisfaction standards and CMS required Pre-Op and Post-Op data collection.

4) Collaborates with inter-professional team members to enhance outcomes across the continuum of care by utilizing, critiquing and/or conducting research to promote evidenced based practice.

5) Identifies high risk patient care issues and trends and implements interventions to achieve optimal/benchmark quality outcomes.

6) Assumes leadership role in population based clinical strategic planning and participates on key committees related to that planning

7) Establishes collaborative relationships with regional providers ( primary care, post acute) and local community organizations to advance population health across region

8) Develops and provides clinical education to direct care clinical staff, cross continuum inter-professional team members as well as patients and families.

9) Participates in community education and awareness campaigns to advance population based health initiatives.

10) Knowledgeable about health care /payment reform and its impact on given population. Participates in creating novel delivery models to meet changing regulations and mandates.

11) Provides concurrent chart review, coordination of care/testing, patient/family education for total joint patient population.

12) Acts as a patient advocate and educator to assure that the patient has the knowledge to care for the patient's condition and patient is educated and empowered to be responsible for participating in the plan of care. Develops individualized patient/family education plan focused on self-management, delivers patient/family education specific to joint replacement program.

13) Provides post discharge follow up and education through telephone/telemedicine

14) Develops and/or utilizes appropriate teaching tools for patients, families and nursing colleagues.

15) Collaborates with nursing and physician leadership in establishing policies and procedures for the total joint replacement program.

16) Maintains a high level of professional standards by participating in continuing education, professional organizations and community outreach

17) Coordinates care between surgeons, anesthesia, PT/OT, case management, perioperative services to provide appropriate continuity of care including utilization management, discharge coordination and planning and obtaining all authorization/approvals/transfers as needed for outside services.

18) Oversees patient education and preparation, resource case management, discharge planning and ensuring patient continuum of care services are met and monitors outcomes.

19) Participates in the development and management of policies, procedures and regulatory requirements to maintain certification/accreditation

Required Work Experience:

1) Minimum of five years acute care experience

Preferred Work Experience:

1) Three years of recent orthopedic surgery experience.
