Join our team of competent and compassionate professionals as a Clinician GSSSI at Behavioral Health Network! The Clinician GSSSI will use a person-centered approach to work with adult individuals who have behavioral health needs, both in short and long-term capacities. Clinicians work collaboratively with the client and outside providers to conduct assessment, crisis intervention, refer and access needed services including support groups, and when needed, provide short term counseling. Your experience, knowledge and great skills will support the program with the following: Work directly with older adults with behavioral health conditions: conduct in-depth behavioral health assessments for high-risk individuals in home or community locations, provide needed assistance, community stabilization and referral to ongoing therapeutic services, as needed. Assist older adults to accept, seek, and navigate to additional behavioral health care services and treatment (ex., Cognitive Behavioral Therapy) and acting as the bridge between older adults and the behavioral health care system. Ability to assess older adults with behavioral health needs, presenting problems, and current mental status. Assessments are comprehensive, and with consideration given to social, psychological, cultural, spiritual, and environmental needs to determine a plan/intervention. Reviews intakes and any additional specific documentation in a timely manner in order to screen and follow up on the referral as appropriate. Maintain caseload of clients with behavioral health conditions who require case management until counseling, connections with a prescriber and/or psychotherapy is arranged in order to remain safely in the community with needed supports. Manages the necessary documentation and submits the information required by third-party payers to secure reimbursement, if applicable. Provide referrals and a hand off to crisis intervention services, as needed. What are we looking for in a Clinician GSSSI? Broad knowledge of BH needs of older adults, brain injury or cognitive impairments and how to access community based and LTSS resources. Master\'s degree plus two years of relevant professional experience required. LICSW, LMHC, or LMFT license (preferred). Valid driver?s license (required). Vehicle available during all working hours. Vehicle should be Registered and inspected in accordance w 540 CMR 4.00. (required) Professional demeanor in communicating with members, colleagues and other providers. Familiarity with Internet, email systems, electronic health records (EHRs), and databases. Excellent written and verbal communication skills (required).
