Coverage Area: Jacksonville, IL | New Berlin, IL | Diveron, IL | Girard, IL

As an Elara Caring Hospice RN Case Manager , you'll rock our world in the following ways:

  • Transform lives : Assess patients' physical, emotional, and psychosocial needs, determining their eligibility for hospice care. Empower patients and their families through compassionate support and personalized care planning.

  • Be a guiding light : Develop individualized care plans in collaboration with a multidisciplinary team. Prioritize patient needs, establish goals, and implement interventions that ensure comprehensive and patient-centered care.

  • Alleviate suffering : Skillfully manage pain, symptoms, and discomfort through evidence-based practices and tailored interventions. Utilize your expertise to bring comfort and relief to patients during their end-of-life journey.

  • Foster healing : Provide wound care and effective management of ulcers or skin conditions. Employ your expertise to promote optimal healing, using advanced dressings and techniques to enhance patient comfort.

  • Educate and empower : Be a trusted resource for patients and their families, offering comprehensive education on disease progression, treatment options, and end-of-life care. Support informed decision-making and provide guidance during emotionally challenging times.

  • Emotional anchor : Extend a compassionate hand to patients and families, offering unwavering emotional support. Guide them through advance care planning, grief, and bereavement, helping them find solace during their journey.

Requirements :

  • Hold a current, unrestricted Illinois state RN license.

  • Bring a minimum of 1 year of experience in a clinical care setting as a nurse.

  • Demonstrate willingness to travel within the branch/office coverage area (up to 50%).

  • Possess a reliable vehicle, valid driver's license, and current auto insurance. Let's hit the road and spread some cheer!

  • Preferred: Hold an Associates or Bachelors Degree in Nursing.

  • Preferred: Have previous Home Health or Hospice Experience. We value experience, especially if it comes with funny stories to share.

  • Preferred: Possess previous experience with Homecare HomeBase. You know who you are, you Tech-savvy professionals!
