Providence St. Joseph Health Social Worker - Rise Program in Spokane, Washington

Description Provides care coordination and basic social work duties to patients/families for an adolescent partial hospitalization and intensive outpatient program. Works collaboratively with behavioral health therapists, physicians, external organizations and community resources to provide needed services and coordination for transitions in the continuum of care. Applies the social work process in contribution to an interdisciplinary approach to assessment, planning of care, and intervention. Facilitates discharge activities and coordinates outpatient services. Responsible for basic milieu management, behavioral redirection and support to adolescents in the program. Individualizes patient services using age related concepts based on the unit population assigned: adolescents (ages 13-18) by applying age related concepts...including knowledge of growth and development, safety needs, collection and interpretation of data, treatments and care typically required and participating in the evaluation of care. Utilizes age related concepts when communicating with patients and families, and when providing counseling or teaching. Providence caregivers are not simply valued - they're invaluable.
