The Diabetes NP will provide specialty level diabetes care in the outpatient and inpatient clinical setting. In this role, you will have the opportunity to be part of an evolving Endocrine program with Atlantic Health System and receive mentorship from experienced providers in a collegial and collaborative environment. The ideal candidate is expected to have 2-3 years of clinical experience- preferably in an acute care and/or physician practice setting with diabetes patients. You will have one year to acquire the Diabetes Care and Education Specialist certification.
Responsible for working alongside physicians and facilitating advanced primary and specialty nursing services. Provides care for adults, perform check-ups, treat illnesses, order lab tests, and prescribe medication.
Principal Accountabilities:
Provides new patient consultation and reviews health history.
Performs physical examinations and screening on the patients.
Provides after-care/ongoing care information and resources to patients and their families.
Ensures patient records are maintained including test results, immunizations, condition changes.
Delivers quality, physician-level care while complying with company policies, safety regulations, and productivity metrics.
Recommends further treatment options or future appointments if required.
Performs diagnoses health problems and provides medications.
Performs other related duties as assigned.
Required:
Master's degree in Nursing or related area.
Current state Nurse Practitioner license.
Graduated from accredited APN program
Must be a Certified Diabetes Care and Education Specialist (CDCES)
Minimum of 2-3 years of clinical practice experience in related field
ACLS and BLS certification
Preferred:
- Acute care/physician practice setting experience
