?Education / Training: Current Pa Nursing License. Diploma in Nursing, associate degree, or Bachelor Degree in Nursing. 

*Experience: *Two years of experence in an acute or long -term care facility or working with Individuals who have Intellecual/ Development Disabilites. 

1. Utilizes the nursing process to promote optimal health of individuals served
2. Develops and implements healthcare goals based on needs
3. Maintains electronic health records for assigned individuals*
4. Prepares nursing 90-day assessment of individuals in the Older Adult program*
5. Prepare annual medical summaries, Health Risk Screen Tool, and other pertinent assessments*
6. Communicates pertinent medical information to appropriate persons, including the program coordinator, families, and day program staff
7. Monitors the hygiene of individuals served and initiates corrective action as needed
8. Attends specialty medical appointments as needed
9. Provides health-related training to agency staff and members of the interdisciplinary team
10. Ensures completion of medical appointments
11. Completes pre- and post-appointment paperwork
12. Obtains and transcribes physician orders
13. Monitors medication distribution, storage, and disposal
14. Coordinates ordering of medical equipment and supplies
15. Assumes responsibility and accountability for own clinical judgment and actions
16. Takes an active role in identifying ethical issues that impact care and advocates resolution.
17. Assists in identifying and obtaining appropriate medical care
18. Provides coverage for nurse colleagues as needed
19. Participates in the nursing on-call and clinic rotation

20. Educates and mentors new colleagues with job tasks
