Finger Lakes Community Health (FLCH) was founded in 1989 with an original mission of serving the region's agricultural workers by linking individuals to area resources and providing essential health services. In 2009 the organization expanded to become a Federally Qualified Health Center (FQHC), serving patients of all incomes, ethnicities and walks of life. Over the last decade, FLCH has grown to provide medical, dental, and behavioral health services to over 28,000 patients at eight health centers.     Job Summary:

The Financial Advocate/Navigator is responsible for educating and assisting individuals in need of insurance with health insurance applications. The position also assists individuals with the identification and successful utilization of health care cost assistance programs.

Duties/Responsibilities:

Insurance Enrollment

  • Schedule appointments with eligible individuals

  • Advise individuals on eligibility for premium subsidies and public health insurance programs through in person or over the phone appointments

  • Enroll individuals into public and/or private health insurance coverage through the New York State of Health Marketplace.

    Referral Base Expansion

  • Establish referral sources though education and relationships with community organizations

  • Serve as a financial contact for all Finger Lakes Community Health providers and case managers

  • Respond to referrals from all sources including New York State, Finger Lakes Community Health, and community organizations

  • Participate in community outreach activities to meet potential enrollees

  • Educate potential enrollees with State-approved materials

    Documentation

  • Report monthly enrollment cases, outreach, and education activities to the grant system

  • Record case details in the electronic health record system and grant system including demographics, service provided, and referrals made

  • Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures

    Education and Qualifications:

  • High School or Equivalency Diploma

  • Prior experience a plus, but not required

  • The ability to work flexible hours -- some evenings are required

  • The ability to communicate fluently in Spanish and English preferred

  • A valid New York State driver’s license in compliance with FLCH drivers’ policy

    Physical Requirements:

  • Must be able to hear and communicate with clients and staff on the telephone and those who are served “in person.”

  • Must be able to lift up to 40 pounds.

  • Must have vision that is adequate to read memo’s computer screen, registration forms, and other documents.

  • Must possess manual dexterity, to perform writing and keyboarding tasks.

  • Prolonged periods of sitting and working at a computer.

  • OSHA Level II
