Location: Holy Cross Health has two hospitals and four healthcare centers all a short driving distance from Washington DC and Baltimore, MD
General Summary:
Serves in an expanded nursing role to collaborate with high-risk, medically and/or socially complex patients and their Primary Care Providers to provide a model of care that ensures the delivery of quality, efficient, and cost-effective healthcare services. Assesses plans, implements, coordinates, monitors and evaluates all options and services with the goal of optimizing the patient’s health status. Integrates evidence-based clinical guidelines, preventive guidelines, protocols and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the assigned patient population.
Develops systems of care that monitor patient progress and promote early intervention in acute care situations. Works effectively with other members of the health care team to optimize interventions.

Minimum licensure/certification required (if applicable):
RN required. BSN preferred
Licensed by State of Maryland as RN. Care Manager Certification preferred
American Heart Association Basic Life Support for HealthCare Provider (BLS/HCP) certification required.
Minimum Knowledge, Skills, & Abilities Required:
Three years' experience in clinical nursing case management with adult medicine in primary care/ambulatory care, home health agency, skilled nursing facility, or hospital medical-surgical setting.
Excellent assessment and triage skills.
Ability to implement evidence-based interventions and protocols for chronic conditions.
Knowledge of chronic conditions, evidence-based guidelines, prevention, wellness, health risk assessment, and patient education.
Knowledge of the basic concepts and principles of managed care.
Completion of self-management support training preferred.
Demonstrates customer focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary health care team, community agencies, patients, and families with diverse opinions, values, and religious and cultural ideals.
Demonstrates ability to develop positive, longitudinal relationships and set appropriate boundaries with patients/families.
Demonstrates ability to work autonomously and be directly accountable for practice.
Demonstrates ability to influence and negotiate individual and group decision-making.
Demonstrates ability to function effectively in a fluid, dynamic, and rapidly changing environment.
Demonstrates leadership qualities including time management, verbal and written communication skills, listening skills, problem solving, critical thinking, analysis skills and decision-making, priority setting, work delegation, and work organization.
Critical thinking skills and ability to analyze complex data sets.
Ability to manage complex clinical issues utilizing assessment skills and protocols.
General computer knowledge and capability to use computer; ability to document/access effectively within multiple electronic health records, health-related databases and other care related platforms.
Reports to: Director, Population Health
Pay Range: $38.18-$55.77
Pay is based on experience, skills and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
