Bellefaire JCB Clinical Case Coordinator: Monarch Boarding Academy in Shaker Heights, Ohio

Bellefaire JCB helps more than 43,000 youth and their families yearly achieve resiliency, dignity and self-sufficiency through its more than 25 programs, including the Monarch Boarding Academy. Our Monarch Boarding Academy is a co-ed therapeutic residential treatment program for individuals with Autism ages 8 through 20. The program focuses on developing communication and life skills so residents can learn how to function better – allowing them to return home or to a less-restrictive setting. Residents live in secure, home-like cottages based on peer relationship skill sets.

Position Summary: Our Autism Residential Services team is hiring a Case Coordinator to be a Supervisor within the Monarch Boarding Academy. In this role, you will provide coordination and oversight of the clinical case management of individuals residing in our Residential Treatment Program.

Responsibilities Include: 

  • Provide oversight, coordination and education related to client case management, treatment planning, assessments, medications, and continuing case requirements.

  • Develop and coordinate treatment plan with treatment stakeholders. Participate in meetings. Coordinate information and case requirements with other case providers. Provide education to staff, families and other stakeholders the case management process.

  • Monitor delivery of case by completing client rounds. Document the case and identify progress toward desired case outcomes. Intervene to overcome deviations in the expected plan of case and review the case plan with clients and family in conjunction with the direct case providers.

  • Interact with involved departments to negotiate and expedite scheduling and completion of assessments, interventions, and consults. Maintain ongoing communication with staff regarding variances from the case plan or transfer/discharge plan.

  • Evaluate outcomes of case with the interdisciplinary team by measuring intervention effectiveness with the team; implementing team recommendations.

  • Assist in supervision and training of staff.

Qualifications:

  • Master’s Degree in Psychology, ABA, Counseling, Social Work, or related field preferred.

  • Bachelor’s Degree required.

  • Experience working with children, adolescents, and their families required.

  • Previous experience with mental health and developmental disabilities preferred.

  • Ability to deal with crisis situations which require prompt decision and action.

  • Sensitivity in relating to persons of varying backgrounds and demonstrate ability to work with diverse groups of people possessing various strengths, aptitudes and abilities.

  • Ability to perform job responsibilities with a high degree of initiative and independent judgment.

  • Practical and creative problem solving skills.
