UW MEDICAL CENTER - NORTHWEST BEHAVIORAL HEALTH has an outstanding opportunity for a PER DIEM SENIOR SOCIAL WORKER - BEHAVIORAL HEALTH (Rotating/Variable Shifts) . The primary role of this per diem position is to support the Intensive Outpatient Program. The Social Worker will be asked to cover psychoeducational groups (curriculum/handouts are provided by the program) and related documentation for sick and/or vacation coverage.

PRIMARY RESPONSIBILITIES: • Utilize medical record and consultation with interdisciplinary staff to screen patients for service needs, provide information regarding Social Work activity and to coordinate care as part of an interdisciplinary team • Assess for the psychosocial and disposition planning needs of patients and when appropriate, their support systems, and develops appropriate interventions to address such needs • Provide patient-centered care by treating patients and their support systems with respect and dignity, involving them in their care whenever possible • Serve as liaison between medical staff and patients/families, providing support and advocacy as needed

REQUIREMENTS:

A Master of Social Work Degree from a program accredited by the Council on Social Work Education and one year of full-time social work experience in a health care setting or equivalent. Experience can include a practicum placement as part of a social work training program and/or employment experience.

LEGAL REQUIREMENT:

Must hold one of the following credentials issued by the state of Washington: • Licensed Independent Clinical Social Worker
