Veterans may need dementia, rehabilitation, long term, palliative and/or hospice care. Nursing Care: Observes, identifies and responds to the patient's needs for care, including medication, equipment-assisted care and patient/family education. Recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. Knowledgeable of elder abuse and the heightened risk for abuse in the geriatric population, observe for subtle signs and symptoms, and reports immediately to the outpatient RN/MD if there are reason or evidence of abuse. Role-models, responds to, and delivers care to Veterans in a respectful, non-judgmental manner. Maintains Veteran privacy in all manner; written or spoken communication or documents, protecting passwords, keeping required education current and intervening appropriately if Veteran information is left exposed. Demonstrates knowledge and skill performing support Duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures. Assists with the scheduling of patient care within the service line and/or medical center. This includes helping schedule Veterans as needed, refers for timely scheduling and after training, might include helping obtain a Primary Care Management assignment and/or entering the Veteran into and electronic wait list. Collects Veteran data including observation of Veteran status and offers information for the plan of care to include: Checks in Veterans with Vital Signs, oxygen saturation, and weights. Completes clinical reminders as appropriate. Recognizes subtle deviations from normal and/or changes in the Veteran's I resident's condition or response to nursing care. He/she reports changes in Veteran's I resident's condition or response to nursing care to the RN or Licensed Independent Practitioner. Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform these treatments according to established policies/procedures. Qualifications To qualify for this position, applicants must meet all requirements within 30 days of the closing date of this announcement. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant] patient information. Completed work should need only a general review by a RN/MD/DO for appropriateness and conformity with established policies/procedures.
