Providence St. Joseph Health Community Health Worker in Seattle, Washington

Description Communicates care coordination information by phone, fax and email in accordance with health system policy. Works directly with patients and their families to address social barriers to health. Ensures member access to services appropriate to their health needs. Conducts screenings and gathers information to identify individual needs for addition to the care management plan. Coordinates internal and external resources to meet identified needs. Monitors effectiveness of the care management treatment plan and escalates concerns to RN, SW, or Provider teams. Interfaces with the clinical care team and community partners in the development of care management plans. Makes home visits to patients at place of residence or other facility (e.g. homeless shelter, assisted living, skilled nursing facility). Arranges for transportation to provider visits if needed. Coordinates community resources to address gaps in care; food, housing, support, etc. Engage with patients as directed to identify social determinants of health. Comfortable working with patient that may have mental illness and substance abuse issues. Make follow up phone calls as needed. Required Qualifications: Associate's Degree Health Related, Community Outreach or Social work related field. Or equivalent educ/experience Upon request: Driving may be necessary as part of this role. Caregivers are required to comply with all state laws and requirements for driving. Caregivers will be expected to provide proof of driver license and auto insurance upon request.
