The UW MEDICAL CENTER - MONTLAKE PHARMACY DEPARTMENT is seeking a highly-motivated CLINICAL PHARMACIST - SOLID ORGAN TRANSPLANT .

WORK SCHEDULE • 40 hours per week • Rotating/Variable Shift

POSITION HIGHLIGHTS • Provide pharmaceutical care for our solid organ transplant services • Be a vital part of the multidisciplinary patient care team • Provide drug therapy consults, participate in treatment protocol development, provide patient/family/staff education, facilitate transitions of care, precept pharmacy students and residents • Provide clinical services in both the inpatient and ambulatory clinic settings

PRIMARY JOB RESPONSIBILITIES • Ensure appropriate drug therapy and nutrition, including indication, dose, drug interactions, and drug allergies. • Evaluate pharmacotherapeutic regimens for effectiveness and adverse drug events, and intervene as needed. • Utilize the medical record and other appropriate documentation systems to communicate with healthcare professionals and to document specific pharmacotherapeutic recommendations. • Provide drug information and education to medical teams, nursing, and dietitian staff. • Provide patient/family counseling and education. Update patient medication lists and reconcile medication orders upon hospital admission and discharge, as well as during clinic visits. • Perform comprehensive medication therapy review and other activities as necessary to ensure safe transitions of care between the inpatient and ambulatory settings. • Monitor and modify patient medication therapy in collaboration with the medical team to achieve desired therapeutic outcomes in the ambulatory setting • Provide primary preceptorship of pharmacy residents and student pharmacists. • Perform the duties of a Pharmacist 2

Strong candidates will have prior experience in solid organ transplant and/or PGY2 residency in this area.

REQUIRED POSITION QUALIFICATIONS Two years of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the experience.

LEGAL REQUIREMENT State of Washington Pharmacist License.
