We are currently hiring a Care Coordinator - Saratoga, to work in our Care Coordination Services Agency. We are looking for someone who enjoys working in a fast paced and team environment. This position is for someone who is a problem solver and enjoys helping those in need. As the Care Coordinator, you would be responsible for:

  • Assess and develop care plans to assist clients in addressing gaps in care and social determinants of health

  • Coordinate the care of those with two or more chronic illnesses

  • Refer and link clients to needed services and resources

  • Work with high-risk populations including clients with, a mental health diagnosis, homelessness HIV/AIDS, Sickle Cell Anemia and/or Substance Use Disorder

  • Ability to work in a fast-paced environment

  • Complete proper documentation

Care Coordinator - Saratoga Minimum Salary: $19.23 hourly

Care Coordinator - Saratoga Maximum Salary: $21.02 hourly

Care Coordinator - Saratoga Schedule: Monday-Friday, 8:30am-4:30pm

Qualifications:

  • Bachelor’s Degree (preferred but not required) in Health, Education, or Human Services

  • 1-3 years of case management experience

  • Experience working with diverse populations
