Basic Function: The Director of Case Management and Social Work Departments is responsible for all daily operations of these departments and is the key link between the HIM department, medical and nursing staff and senior management. This person in this position will ensure clinical documentation adequately reflects resource management and severity of illness of the patient population(s). The Director provides the link between compliance, clinical management, patient progression, gathering and reporting of utilization data, community outreach and as a resource in the prevention of readmissions. Requirements: Active NYS RN Licensure and BS or BSN Degree required. Masters' Degree in a related field is preferred. Candidates must have a minimum of five (5) years' leadership experience in Case Management, Utilization Management and Quality Assessment, or a minimum of five (5) years' leadership experience in a related field. Eligible for or certified in Case Management is desirable. Extensive knowledge of all CMS regulations and conditions of participation are essential. Responsibilities: • Responsible for the hiring and managing of all the CMSW department staff. • Assess and reasonably manage variables contributing to the reduction of length of stay (LOS) and cost per case through resource management. • In cooperation with the HIM department responsible for improving physician documentation to adequately reflect the severity of illness of the patient population, wherever possible.
