Maintenance and review of patient medication profiles; reviewing and verifying physician medication orders and patient medical record. Interacting with healthcare professionals and providers to help promote safe medication use, prevent inappropriate or misuse of medications; provide clinical pharmacy services (e.g., pharmacokinetic dosing, therapeutic interchange, IV/PO conversions, clinical protocols per pharmacy and pharmacist interventions). Hourly Salary Range: Step 1, $85.87 - Step 9, $100.58 (Step based on years of experience)
Bargaining Unit: HX - UPTE
Qualifications
We are seeking an exceptionally-skilled and compassionate pharmacist with the following qulifications:
Doctor of Pharmacy Degree
California Pharmacy license
ASHP-accredited PGY2 oncology residency or at least a minimum of 2 years of pharmacy practice experience in oncology
Recent clinical oncology experience (within 5 years)
Board Certification in Oncology (BCOP) preferred
Previous infusion pharmacy experience preferred
EPIC/Beacon experience preferred
Exceptional communication, interpersonal, organizational and follow-through skills
Knowledge of and ability to perform age specific and disease specific pharmacokinetic calculations
