HOME AWAY FROM HOME, CARE GROUP, LLC Direct Support Caregiver (Sandy Heights location) PART-TIME in SANDY, Utah

Daily:

  2. Treat the Clients with dignity and respect at ALL times.
  3. Ensure the Clients' safety, privacy and well-being at all times.
  4. Ensure that Clients' Human Rights are NEVER violated.
  5. You will work with male and/or female adults with disabilities and your absolute professionalism will be expected at all times!
  6. Comply with new DHHS Final Settings Rule https://dspd.utah.gov/settings-rule/#requirements.
  7. Keep the Home clean (wash dishes, sweep and mop the floors, clean shared bathroom, take out the trash, wipe down counters/tables/dining chairs, clear out the dishwasher, wipe the microwave/stove/fridge, throw away any expired food in the refrigerator and assist with any laundry as requested by clients.
  8. Provide Active treatment with the client's during waking hours. (Don't let them just sit in their room or watch TV all day)
  9. Complete required daily documentation (Shift Report, Behavior data, Medication Administration Record, Other Individual-specific documentation necessary for the well-being of the clients we serve).
  10. No unauthorized visitors allowed inside the home (no exceptions).
  11. Give a verbal report of your shift(s) to the oncoming staff.

Weekly:

  1. Ensure clients attend appointments as necessary.
  2. Ensure clients attend work schedules or participate in Day Program during weekdays with the exception of Holidays and weekends.
  3. Wash mop heads and dish towels twice a week.
  4. Facilitate approved visits with family members and friends.

Bi-monthly:

  1. Complete attached "Bi-Monthly-timesheet" fillable PDF and send it to me via email for payroll processing every Monday of pay week by noon. 

30day/60day/90day/180day/Annually:

  1. Complete the required staff training within the required timeframe.

  2. sign-off on client-specific training.

     

Preference will be given to prospects that are CURRENT in any of the following certifications: 

  • CPR (90days to complete)
  • Food Handlers Permit (30 days to complete)
  • First Aid (90 days to complete)
  • Bloodborne Pathogens or Prevention of a Communicable Disease (90days to complete)
  • Defensive Driving (60 days to complete)
  • MANDT System training (180 days to complete)

Must be able to pass State and Federal background check.
